Section 125 Open enrollment Draft:

The deadline for enrolling is December 5, 2006, for 2007 calendar year eligible expenses. Here are three PDF documents outlining the enrollment process and also provide more information regarding what expenses are FSA eligible.

    To communicate the election process and provide a brief overview of VitaFlex eligibility requirements.

  2. 2007 Fremont VitaFlex Election Form
    We encourage employees to elect FSA by using the online system. Should you have trouble you may fill out the paper election form.

  3. 2007 Fremont VitaFlex Plan Detail Sheet
    Your Plan Detail Sheet provides an overview of the specific details of your plan.

Instructions on how to access the FEA account:

  1. Go to Vita Companies website

  2. Click on "Log On", then under Benefits, click on "Employee Logon".

  3. Type in the User ID "fremont" and the Password "fremont", and click "Logon"

  4. You can view or download the following:

    • Plan Information (Medical, Dental, Vision, Life Disability, and EAP)

    • General Information (Complete Summary, Eligibility, and Forms
